Clinical relevance

This assay enables the quantitative determination of 53 amino acids and metabolites in urine including the parameter creatinine, so that the results can be directly related to the creatinine value without further analysis. It is intended to be used for patients with suspected metabolic diseases in whom the urinary amino acid and metabolic product levels are of clinical importance, specifically as an aid to

  • diagnosis of cystinuria
  • diagnosis of lysinuric protein intolerance
  • differential diagnosis of Hartnup disorder versus other aminoacidurias (cystinuria and generalized aminoaciduria)
  • monitoring of treatment efficacy in previously diagnosed cystinuria patients.

Assay characteristics

A differentiated amino acid profile can be created in less than 20 minutes. Sample preparation consists of a fast and easy urine dilution. The method does not require derivatisation, making it robust and time-saving. Isobaric compounds are separated chromatographically. Each of the 53 analytes is safeguarded by its own internal standard compensating variations in recovery, matrix effects and ionisation effectiveness. The 3PLUS1® Multilevel Calibrator Set and MassCheck® controls are produced from synthetic urine to ensure that their composition approximates a patient sample as closely as possible. The assay is available in 2 configurations, either with reaction vials or with 96 deep well plates.

Sample Preparation

  • Pipette 20 μl sample/calibrator/MassCheck® control
    + 50 μl Internal Standard Mix
    + 600 μl Dilution Buffer into a reaction vial.
  • Mix for 30 s (vortex) and centrifuge for 5 min at 16 000 x g.
  • Transfer 200 μl of the supernatant into an autosampler vial.
  • Inject up to 6 μl into the LC-MS/MS system.
